  • Perform initial nursing assessment and establish a plan of care
  • Consults with physician for appropriate orders and communicates condition changes for medical intervention
  • Educates the patient/family at initial visit
  • Records accurate and timely documentation while working with a scribe.
  • Ensures medical equipment, supplies, and services are available for patients
  • Performing wound care or ostomy care/education as needed
  • Educating patients on new medications
  • Ensure patients can safely take their prescribed medications
  • Education patients on new diagnoses
  • Additional duties as assigned

Education:

  • RN License

Experience:

  • Home Health: 1 year (Preferred)
