- Perform initial nursing assessment and establish a plan of care
- Consults with physician for appropriate orders and communicates condition changes for medical intervention
- Educates the patient/family at initial visit
- Records accurate and timely documentation while working with a scribe.
- Ensures medical equipment, supplies, and services are available for patients
- Performing wound care or ostomy care/education as needed
- Educating patients on new medications
- Ensure patients can safely take their prescribed medications
- Education patients on new diagnoses
- Additional duties as assigned

- 8 hour shifts

Education:
- RN License
Experience:
- Home Health: 1 year (Preferred)
